THE NOMINATION FORM IS AT THE BOTTOM OF THIS PAGE

This is your chance to choose that teacher, student or volunteer who you believe has made a difference at Field.  Read Mr. Andrew's story below and get inspired.  Helping others is a gift and it is time to recognize all the wonderful things that people have done to make Field School the best that it can be!

Please submit your nomination to the Field PTA by May 12th.  They can be dropped off to the Field PTA mailbox in the school office or emailed to the Field PTA

Nominating Criteria for the Mike Andrew's Award    
 
Positive Attitude + Actions =  Good Deeds
  • The nominee demonstrates a positive attitude.  
  • Their actions are helping to making Field School and the surrounding community a better place to live.
  • Their actions make it possible for others to recognize and embrace the positive things around them.

 
 

 

Who Can Be Nominated for the Mike Andrew's Award?  
  • Field Teacher and Staff, Field Student and Field Volunteer

     

Who Can Submit a Nomination?

  • Anyone one in the Field School or the surrounding community. 
  • Field Students, Field Teachers and Staff, Field Families are all encouraged to submit nominees!     

 

How do I Nominate Someone?   

 

In 250 words or less

 

  • Describe how your nominee demonstrates a positive attitude.
  • Give an example of how their actions has made a difference in the Field Community.
  • Explain how your interaction with this person has inspired yourself and others.   

 

You must include the following information for your nominee to be considered!  

 

  • First and last name of nominee
  • Indicate if your nominee is a Field Teachers or Staff Member, a Field Volunteer or a Field Student.

 

Where do I Turn in My Nomination?

 

Drop off your nomination to the Field PTA Mail box located in the school office or email your nomination to the Field PTA
